Food Application- Annual and Temporary

Starting in 2023, fees will be charged for food permits issued by the Sandisfield Board of Health

See below for additional information on the following items:

• Workman's Compensation Affidavit required for all permits
• Chokesaver is required for all establishments with 25 or more seats:  Contact BOH for information on local providers
• Allergen Awareness is required for all establishments selling food for immediate consumption
• Allergen Awareness Poster must be hung in employee areas
• Food Manager's Certification is required for all food establishments serving food that is potentially hazardous
